Key Projects

Everyone has the right to basic healthcare and educating and managing people with chronic conditions is an effective way to stabilize overall health care in India. Chronic Care Foundation strongly believes that to make a meaningful and significant contribution; there is a dire need to create awareness and educate the community on the aspects of care and management of Non-Communicable Chronic Diseases (NCCD), enabling the community to lead healthy, active, and productive lives.

Chronic Care Foundation has initiated following programs and tools to reach out to the individuals, groups and communities.

PUBLIC HEALTH EDUCATION CAMPAIGN ON CHRONIC DISEASES
This project comprises of series of educational activities and community based advocacy programs that promote messages about the seriousness of Non Communicable Chronic Diseases (NCCD) to the population at large. These activities stresses on imparting simple, friendly and clear messages on the ravages of chronic diseases, their cause and risks, effects on the individual and in turn their families as well as remedies, management and most of all prevention.

PATIENT & PUBLIC FORUMS
The  Patients  &  Public  Forum  is  a  network  of  national  and  regional  individuals  and  select organisations concerned with the healthcare interests of patients and their families. CCF has three types of Patient & Public Forum

Patient & Public Forum - Diabetes
RESEARCH

The base of all CCF’s activities are supported by Research. CCF conducts niche research in the health sector. It has already conducted a pan India research on “Non Communicable Chronic disease in India: A study of the gaps, quality and cost of care” which has been successful in contributing to the body of knowledge, and also generating further awareness on what needs to be done by all stakeholders. Other research projects are in the pipeline.

SPECIAL PROGRAMS AND EVENTS
I Have A Dream Contest: CCF with support from Baxter India had conducted this innovative contest for the patients who have been suffering from ESRD (End-Stage Renal Disease). This contest aimed to bring a sense of hope in the lives of the patients to continue with life even if they are diagnosed and living with end-stage renal disease (ESRD).
PROGRAM FOR YOUTH AND CHILDREN
CCF reaches out to young and vibrant youth and children of our society through innovative and interesting events and tools. Under this program CCF conducts the following
Quiz programs:  through such programs the information about non-communicable chronic diseases is disseminated in a participatory manner.
TRAINING AND CAPACITY BUILDING    (PDF, 327KB)
CCF designs and implements training programs on capacity building for programme coordinators and frontline workers on prevention of chronic diseases.
